## 4. Quote questions

Ask each provider to answer in writing:

1. What exact itinerary and document version does this quote assume?
2. Which items are mandatory, conditional, optional or excluded?
3. Are consultation, examination, administration and official charges separated?
4. Who verifies the destination and return requirements?
5. Who submits the NOI and books the government appointment?
6. What happens if the veterinarian cannot certify fitness to fly?
7. What happens if a test is delayed, invalid or needs repeating?
8. What fees apply after a flight change or expired validity window?
9. Are after-hours, courier, translation, crate or payment charges included?
10. When does the quote expire, and what is the cancellation/refund policy?

## 5. Schedule and final reconciliation

| Task | Validity or official deadline | Booked date/time | Completed date | Document/reference number | Checked by |
|---|---|---|---|---|---|
| Destination requirements obtained | | | | | |
| Return plan reviewed | | | | | |
| Microchip and identity check | | | | | |
| Vaccination/test/treatment | | | | | |
| NOI submitted | At least 10 working days before departure or isolation under cited Australian guidance | | | | |
| Final veterinary examination | | | | | |
| Government pre-export appointment | | | | | |
| Export permit issued | Export within 72 hours after issue under cited permit condition | | | | |
| Transport check-in | | | | | |
| Return permit/preparation | | | | | |

Before departure, compare the cat’s name, microchip, dates, flight, destination and signatures across every document. Keep the Australian export health certificate and certificate number for return planning. Keep export records for at least two years under the cited department guidance.

## Safety and accuracy notes

- A quote or NOI approval is not a guarantee of certification, export documents or boarding.
- Do not backdate, reuse or alter documents.
- Do not sedate a cat unless a veterinarian who knows the cat specifically advises it; airline rules still apply.
- Domestic airline certificates and international export health certificates are different processes.
- Requirements and prices change. Record a fresh verification date before relying on this worksheet.
